Life Services
When authorized by the team LIFE provides all needed preventive, primary health, acute and long term care services so that qualified (or eligible), older individuals may live in their own homes and communities as long as possible. We believe that older adults with chronic care needs do better when they are able to remain in their homes & in their community.

- Transportation to and from our centers
- Medications ordered by our physicians
- Nursing care
- Rehabilitation and restorative therapies
- Personal care and homemaker services
- Primary and specialty medical care
- Social services
- Social activities and recreation
- In-home care
- Caregiver support
- Home modifications
- Continence Management
- Hospitalization and nursing home care when needed
- Other specialty services and equipment when needed
Many of these services are provided at the LIFE adult day center. Members receive full medical and preventive care, and may participate in the range of activities offered each day.
Care is provided by a team of licensed professionals who work to meet the individual needs of each member. The team remains involved in the member's care even if they should need to be hospitalized or if they require short or long term treatment in a nursing home. The care team is made up of primary care physicians, nurse practitioners, registered nurses, social workers, therapists, personal care workers, and other dedicated staff.
For individuals who qualify Medicare and Medicaid will pay for the member's care. Most members who have both Medicare and Medicaid pay nothing for the program. In other cases, members pay for a part of their care or all of their care if they pay privately. LIFE is both a Health Plan & a Health Care Provider, all needed care, other than emergency services, must be provided by LIFE. The member may be fully and personally responsible for the costs associated with any treatment not authorized by LIFE or received outside of the LIFE program, except for emergency services.
LIFE delivers high quality services and the staff takes pride in what we do. The quality and safety of LIFE services are continually monitored by the program's own Quality Management Program as well as state and federal agencies.
