Is LIFE for You?

You may become a member if you:

  • Are age 55 or older
  • Have health problems that make it hard to live at home
  • Want to remain in your own home
  • Live in zip codes 19103, 19104, 19121, 19130, 19131, 19139, 19142, 19143, 19151 or 19153
  • Need medical care and supportive services and are able to live in the community safely with the services provided through this program.
  • Are eligible for Medicare and/or Medicaid or able to pay privately

LIFE welcomes you to contact our enrollment staff at 215-573-7200. Our staff will be happy to answer questions about membership, and will help you through the enrollment process. You can also tour the LIFE adult day center and meet the staff. Our staff can also help you with referrals to other local services if LIFE is not right for you.
